Prerequisites for JSD Request Types

I would like for all request types in my JSD project to include some kind of questions/check boxes that the customer would need to fill out, and have the priority of the ticket determined by the answers provided/selected. 

Is this possible? I do have Automation Lite installed in my instance to flag certain verbiage within a ticket, but I can't find a way to have these prerequisites included in my request types.

You should be able to. Depending on the details it might be a rather lengthy automation. 
it sounds like you want something like this...

trigger - Issue  created

conditions: if/else block

if RT = A and field 1 = xxx and field 2 = yyy then set priority to high

else...

I only have the free version, Automation lite, I'm still using Server.. but thank you for your input, I'll consider the Pro version. What I did  was create a custom field with check boxes. Then used the JQL option for the condition to flag a certain checkbox to determine the priority.
