It has come to our attention after reviewing some tickets, but users are able to leave internal comments on all service desk tickets.

Applied the suggested permissions, and only internal comments are presented to the users/reporters of the ticket.

The functionality that allows public comments to be entered in jira service desk project tickets is limited only to users who have a jira service desk license. The add comment permission only enables you to create internal comments to make public comments, you must have access to the service desk application or belong to the project role Service Desk team.
If this does not help you can send a screenshot of your permission scheme and the groups of one of the users who cannot comment
