The create request page is not getting loaded with the fields like Priority, label, approvers so on

Just for us to better understand and help you with this issue, can you please give us more details about where this issue happens? Is it when an agent creates a ticket or when a customer creates a ticket?
Are you using any add-on for service desk forms?
Have you tested using another browser or incognito mode?
Also, just to confirm, are you using Jira Cloud or Server?
