Can Project Widget be customized to include more fields?

Hi Andrew and welcome to Atlassian Community!

Currently, it's not possible to add more fields on the widget. It will only show the fields added to the request types available in the project for the customers to fill. We have a feature request suggesting the implementation of this ability:
