Include a Fillable PDF Forms in a Request issue.

Scenario.

For our project, the requirement is that any request coming to a particular group should have fillable pdf form. the assigned member should download the form, fill it and  submit to the next level.

I knos this is possbile...but how do i acheive this? Looking for suggestions.

We have server request types that we link forms to in the Request form help and instructions.  We asked the user to download and complete the form then attach it as part of the request.  We also make the attachment field required and place instructions on it as well. This works well for us.
