A couple years ago we turned on notifications and our customer base was flooded with notifications by email, so we kept them off.
I wish to turn them on again and am afraid of it happening again. What's the result if I turn them on now?

You should turn on the basics, e.g. issue created, issue resolved and of course public comments.
Is there any retroactive items that get sent out once I turn on the switch, or does it start fresh, only new notifications will be sent?

No. Notifications are sent at the time of the event so all things that have happened previously will not trigger an event.
