I have moved the request type in all the screens to the top above issue type. Still when adding a new issue the issue type is on top. What am I doing worng?

What are you trying to achieve? The Issue Type is stored on the ticket by the Key at the very top of the page, whereas Request Type is stored within either the Description or Context Fields.
The order of fields on Screens don't matter too much, the order and position of the fields is set on each Request Type individually.
On each Request Type, go into the Work item view tab and there you'll be able to move your fields around to your desired format.
