How to create a template for a case (Vorgang)?

I would like to create a template with single fields, which I can add as a new process.

E.g. a ticket template with pre-defined fields:

• Ticket No.
• Modell
• Actions to be performed with tick boxes to be selected
• free text field
• dropdown

How can I create such a template?
