How do you embed an image in a JSM form

I have a JSM form with multiple fields for a user to complete. In one of the fields I want to show them an example of an answer that includes an image. Is it possible to embed images in a form?

Hi @Kathleen Hamilton 

Unfortunately from what I know in JSM embedding images directly into form field descriptions or help text is currently not natively supported in the form builder for request forms (especially the newer ProForma forms or native forms in the portal) 

Maybe adding a link to an image might work as a workaround?

In Jira Service Management’s native forms, you can use help text or field descriptions to show examples, but these currently only support plain text or basic formatting — embedding an image directly in the field isn’t available.

If you want to include visual examples or reference images inside the form, another approach you might consider is using Smart Jira Forms (developed by our team). It supports embedded content blocks, so you can place an image, diagram, or even a short video right inside the form — either as a standalone content element or next to a question.

For example, you could:

  • Add an “Embedded Content” element with a product screenshot to show users what their input should look like.

  • Include an image or short GIF between questions to visually guide them through the process.
