How do I fix a Project Creation Error in Jira Service Management?

Folks,

I keep getting a "Hmm... we couldn't create your project due to an unknown error. Try refreshing the page to start again." when I try creating a project for Jira Service Management.

Wondering how to fix.  Please and thanks.
