How can you show a specifik form based on what your second pick is in a select list(cascading)?

Hi,

Say I have a select list and based on what the customer chose on the second list I want to have a customized form after that value for the customer to fill in. How can that be done?

You can't do this without re-writing service desk.  The request is intended to be a simple single page for the customer to enter all the information asked of them.
