I'm trying to see if the HR project in Service Management is going to fit our needs. We're trying to create the below simple process:
Once I get passed this sticking point I've got the rest, just can't seem to wrap my head around the formidable forms creating a card with the custom issue type I've created.
Can what I need be done, or is there a work around that anyone knows?

It looks like Formidable form allows you to send REST API calls, so this should be doable. Just note that the REST API for JSM only allows you to create a request, it does not allow you to update one. In order to trigger the email from JSM to the customer they have to be added to your project first.
I also saw that Formidable form integrates with Zapier, so yes this can be done since Zapier integrates with Jira.
