Forms with user evidence (photos) input

Hi!

I'm working on a project where the support team must upload some photos linked to a certain ticket.

Currently I'm working with a 3rd party app (Lumiform) to upload the required information, but it's getting hard to track all the forms.

I'm looking for an alternative where I can create a form directly linked to an issue in Jira, and that te person filling up that form can upload pictures taken in their phones, regarding the issue.

Is there any tool like that that I can work with Jira?
