Forms: I only want a form to generate in JIRA if a particular field is selected.

I have created form, however, i only want this form to appear if an answer to a previous question is 'YES'. Is this possible?

I don't think his is possible right now. The forms are only displayed based on the request type selection.

One recommendation is to have different request types based on your main condition you mentioned above.

The alternative is to post a feature request to atlassian.
