Is there a way to program the Jira Service Management widget with required fields such as URL, name, email?

This can't be done.

"If you see an error saying the request type isn’t valid, it means the request type has a required field that can’t be rendered in the widget. Make the required field optional to use that request type."

I agreed with @Marc -Devoteam- suggestion on widget where the required fields are controlled in the Request Type customization in the JSM project side.
