Customer Experience Portal site not working...Forms not showing up

I just opened a new Customer experience site and it is not working...How to get the customer input form to show up so customers can input their issues?

Welcome to the community.

IF you have setup a JSM project and created ac form, ac form needs to be linked to a request type, existing or new one created.

Welcome to Atlassian community.

You need to create a new service project, you can create a new one based on one of template offered by JSM.
