Create 3 conditional fields.

Hello guys, I'm trying to create 3 conditional fields, where it would be according to the example below:

Field 1 (dropdown) - Select your country - Example USA

Field 2 (drop down list) - Select your State - CA Example

From the above combo, it would have to appear 2 text fields to insert:

Zip code

ADDRESS

And there could be more than one combination of field 1 and 2 and the text field information is requested according to the result of the combo.

It is not possible without addon you will need NFEED for example.
