Can I fully customize design for knowledge base (customers portal) like https://help.medium.com/hc/en-us, https://help.twitter.com/en, https://www.facebook.com/help/?ref=pf.

I want to use my own design to insert in Jira Service Desk.

Not natively, unless you want to pull apart the service desk application and rebuild it after hacking all the layout.

Have a look at the Refined Themes for Service Desk though, that can go a long way to customising the design
