I have multiple external customers and an internal organisation that I would like to serve from a single instance of JIRA Service Desk 4.2. The ideal scenario for me is:
Customer X, Y Z and the internal organisation have different Customer Portals and we manage them through a single Service Desk project. Is this possible.
Unfortunately you can create one customer portal for one JSD project.
You can achieve what you want by using Extension for Jira Service Desk add-on (Visibility feature)

I appreciate your feedback. If I were to use this add-on would that impact would there be any issues when Atlassian update/upgrade the Service Desk product?

Using any add-on you should be aware that add-on's compability can blocked any upgrade. But I feel that you know this.
But using Deviniti add-ons chance for that is very low. In my career I had some add-ons from Deviniti and never had problems that any something block my upgrade.
Also support is very good so every problems/questions are quickly resolved/answered.
