I am trying to set up automation on a JSM request type. The request type has a drop down for what issue the user wants to raise a request on behalf of. The form is only needed for one of the issues in the drop down. Is there a way to attach a form on the request (as they are filling out the request)?
I.E... I chose "Website" in a drop down that has 3 other options. I only need the form to be attached and fill out, when I choose "Website". I need the form to attach and fill out before the request is submitted by me.

In the form attached to the request, create sections, so that based on the drop down option chosen the required fields will be shown.
