The automation keeps creating a new issue resulting in duplication when the transition is triggered. I only want one issue created in the Jira Software / another project the first time the issue is moved from review to work in progress. The next time the transition is triggered, it shouldn't create a new issue.
Any guidance would be much appreciated.

Can you please post your audit log from the rule? In addition this IF component comes with the option to "restrict it" on one project. Can you confirm that the linked issue is on the same project as the one being transitioned? If it is restricted to just one project like in the image below, and you have a linked issue on another project, then your rule will not work. You have to change the rule's scope.
