Add comment to a ticket when a field or form is updated

We have a form that users are able to update,  we have an automation that sets the status to "Changes Made" when this happens, but I would like the changed field to be explicit to the Service Desk agent rather than trawling through the history on the ticket.

Is there a way of adding any changed field as an internal comment by automation?
