Is it possible to require 2 faktor when customers create their user for the first time. Can I somehow manage a policy that requires this when users login?
I'd say yes for managed users' Atlassian accounts, not portal-only customers. In this case, you will need to verify your domain, subscribe to Atlassian access, and configure two-step verification and authentication policies.
Can I use this approach for all my customers and move them one by one?. We are running a B2B business, which means it for the time being can be a manageable solution. As long as I dont need to by a user licens for the users (customers) that only access the customer portal.
Will this require a subscribtion for Atlassian access for all my internal users or only the ones that uses the service management solution?

You can, observing what I mentioned initially. For the second part of your question: Only JSM agents will be counted towards your Atlassian Access bill. Portal-only customers are not billable for Atlassian Access unless they have product access to another Atlassian Access supported product. For more details, see Manage your bill for Atlassian Access.

Hi @Jørgen Østergaard - Unfortunately, the provisioning and operational setup for customers are still a pretty basic username/password at this time. Atlassian does have a longstanding request that would provide the ability to integrate Organizations/Customers with an Identity Provider which would allow you to manage things like enforcing MFA.
