How do I change the title of my form

I have a form with the title "IT Support Ticket", but when I go to see the form on the site, the title shows "Contact us". I can't find where I can change this header on the form. What am I missing?

Is this for JSM cloud or DC?

If it's cloud, that's either a Banner or a Help center announcement

Check the Help center customization and remove the announcements.

This is JSM cloud and it's a form in the Customer experiences section

Customer experiences > Forms

and this is visible on the Support site of that customer experience

Customer experience > Channels > Support website

I did find the Help center, but I didn't even know I had that and I haven't touched it. Is the Help center somehow linked to Customer experience ?

Thanks for the clarification that you are using a Customer Service space.

The help center can be found for other JSM spaces. Customer experience its only for the customer service space.

Check the support website page-topper (might be too big or something happened to it that its blocking the view) or the branding.

Go to channels -> support website for the page topper.

Try taking it out first then modify it, make it smaller.

When I go to channels -> support website, I don't see any thing that indicates I can change the page topper... When I go to channels -> support website -> customize website, I can only edit the homepage and that doesn't have any page topper on it. 

The Pages tab in channels -> support website doesn't show I have any pages. It seems like the Contact Us page is the only page I can't customize (outside the form) and it seems the form titles are only used for the dropdown to select a form
