Why does my page layout change from left align to center?

I was going to edit my page and suddenly the layout change from left align (view mode) to center (edit mode). Once I saved, it became center on view mode as well. Also some images became broken. This happens on several documents only.

I noticed on the broken documents that when I change to edit mode, url change to /edit-v2/. While on normal document, url is /edit/. 

Anyone ever experienced the same case? Is it going back to normal if I edit the url from /edit-v2/ to /edit/. I'm afraid that would harm the document and I might lose the document.
