For each page within our Confluence space, I want to have a field where a user can provide input/feedback and the authors receive a notification so we can make an update to the content. Something like what you provide here: https://confluence.atlassian.com/conf62/collaboration-902074673.html
I haven't found an available macro that can do this.

The screen shot in the linked question shows the add-on working with Confluence 6.2. If you have that version (the article you linked is for Confluence 6.2) or an earlier version then it should work for you. If you have a later version of Confluence please let me know which one and I can research further.

Hi Susan,
I tested the plugin version 1.4.2, downloaded from the Knowledge Base Survey Plugin page under the Version history heading,
I uploaded it to my Manage Plugins page and then set up the questions under Space Tools>Knowledge base Survey. Here is the result I got on Confluence 6.8.x:
Please try that strategy and see if it works on your instance.
