It looks like this plug-in is no longer supported.

• If you click on the Documentation link in the macro browser for the KB Survey macro, it takes you to a "Page unavailable" message.
• The Marketplace page for the plug-in says "This app has been archived." ... and gives a Version history that ends in 2014 with no support for anything higher than Confluence 5.6.
• That Marketplace page has a link for "Read the documentation", which also says "'Confluence Knowledge Base Survey Plugin' has been deleted."

So, it appears to be dead.
