Jira Issues Macro, custom field in create new issue

I have a confluence space linked to JIra.

We require some of our custom fields to have input during this process.

I found this article about adding a velocity template to the field definition but could not figure out where to add that.

Anyone have any step by step guide to adding custom fields into this screen

You have to set those CustomFields Required in the Field configuration scheme your Jira project.
