How do I include the feedback on a page?

Hi, I would like to include a feedback option on a page that allows the users to provide their feedback if the article was useful or not. Also, I want to make it customisable so the user can enter their suggestions. Is this attainable if I modify the space to allow for Global users. In addition to this, I do not want to the comments by other users to be visible to other users.

For the feedback you can try this plugin though it was disabled for all instances past 5.6.6.

I think there is some javascript in that answer that works, but I haven't tested it.
