Confluence plugin for easy build questions and answer system?

We are using Confluence for documentation and I am trying to document incident resolutions. I would like to narrow down the type of incident to present the correct resolution by asking a user a list of questions. So basically sort of multilevel questionnaire where the system ask a question and based on a list of predefined answers asks more questions and in the end comes up with a resolution.

Are there any plugins for Confluence that allows me to easily manange these questions-answers-resolutions?

I can recommend the Survey plugin. It's the one we are using on our Confluence and it's pretty good.
