How can I automate the date and member who checks the task to be filled in automatically every time a task is completed?

You can do that through automations.
Assuming a task is completed by being moved in to the Done list (though it could be anything, if you've a more esoteric setup) you can use that as the trigger.
Something like below.

I tried this, and it adds the member to the card. It doesn't show that the member has completed the checklist item.
Any other ideas?
