New Issue Type: How do I create different screens for different issues types?

I want to keep the Issue Types that come with JIRA (Bugs, Enhancements, etc.), but I want to add "Notes" and "Decisions". Each one of these new types would have different screens.

How can I accomplish this?

Thank you (as you can tell I am a novice and I have not attended classes, due to budgetary reasons)
