I've found the "Average Time in Status" report, but first off it doesn't seem to ever return anything useful, and secondly, it seems to have an axis that isn't very useful. What I want to know is, for a set of issues determined by a filter, for each status or step in a workflow, how much time was spent in that status?
The Y axis would be amount of time spent. The X axis would be each status or step in a workflow. This allows me to understand where my team is spending the majority of their time.

There is a Time in Status Plugin on marketplace. You can get reports depending on user, project and advanced mode. You can export the reports to excel also.

Are there any examples out there how others have built more effective "issue time in status" reports using these methods?
We are trying to ask JIRA some basic questions about time in status: by status value for the past N months, the average time in status for <issue type> for <project> is X days.

Since you mentioned that the current Average Time in Status report is not able to cater for your requirement, perhaps you might want to look at the possibilities of either customising your report via developing your own report (https://developer.atlassian.com/display/JIRADEV/Report+Plugin+Module) or even the usage of report-related third party plugins such as PowerReport (https://marketplace.atlassian.com/plugins/com.valiantys.jira.plugins.reporting) or JIRA Timesheet Reports and Gadgets Plugin (https://marketplace.atlassian.com/plugins/jira-timesheet-plugin).
