Fix Version and Affected Version not on Default form

The fix version and affected Version fields have been removed from my default form. Is there any way add them back. I have already tried "Add Field" under administrator but it is not there.

you go to the screen which you are using Admin->Screens->{Select your screen}->Configure->{get those from drop down list at the down of the screen}

Add Field is for adding new custom fields to JIRA in the context you are already in. In this case, you will have to modify the screen schemes as Bharadwaj suggested. See https://confluence.atlassian.com/display/JIRA/Defining+a+Screen
