Error creating Jira issue in vscode Atlassian extension

I've installed version 3.0.15 of the "Jira and Bitbucket" vscode extension.  I've managed to setup the extension so that I can view existing issues.  The problem I'm running into is when trying to create an issue.  A window comes up and says "No fields found for selected project. Please choose another."  No matter what project I select, pressing 'submit' results in a blank screen.

If I enable debug, I see "Error: error creating comment: AxiosError: Request failed with the status code 400" in the log.

Anyone know how to resolve this issue?

Welcome to Atlassian Community. 

We can check the below things firstly. 

1) Try to check whether you have the issue create field or required fields in issue create.

2) Try to check the project settings,

3) Check the permissions of the project and user permission

4) check vscode configurations, project key's used

5) try to enable or disable vscode and try. 

let me know if all the above are checked and correctly configured.

I'm very new to Jira and unfortunately don't really understand what I need to look for in your first list item.  Can you help me understand this better?
