I'm trying to add a custom field (text box) to the request type form and when I select Add Field option, I don't see the field as an option.
The custom field is not hidden because it is available on other screens.

So your custom field must be on the create screen of the screen scheme for the issue type that is related to the customer request type in order for you to select it.
If it is and you still can't see it, then check the custom field configuration to see if there is any contexts on the field. You might need to add this project to a context for the custom field.

I always forget about field contexts.
Had a custom field that did not appear on a request form, but was on the create screen. Removing the context and making it global made the custom field visible and resolved the issue.
