I read somewhere in this forum that it would be possible to setup issue subtypes using script-runner add-on. What we need is having issue types categorised into subtypes like if issue types are - Incident, Service Request, Change, we would like to get additional options when you choose an Incident, for example:
Incident > hardware incident
Incident > Software Incident
etc.
Have anyone ever set this up properly? or maybe you could give an advice on how it would work / set up required for it?

Are you asking for custom Subtask issue types?
If it is subtask issue types, then it can be created (doesn't required ScriptRunner) out of the box. However, it is only available once the parent issue is created, then one can create subtasks for it (i.e: select your custom subtask issue types once those issue types are added to your project's issue types configuration scheme).
For regular issue types, there is only two categories that you can do - Standard or Subtask. It is not possible for one to associate a custom "Standard" category issue type to be listed under as an option of another standard issue type as far as I know of it.
A possible option for you is to tag your issues with custom field, thus when an "Incident" issue is created, there is a custom field that can be tagged with the subtype option value. Or, you can even use "Component/s" setup for the tagging process if you don't want to create new custom field.
