As per the CSAT section, I can see only one question-- "How was our service for your request?"
Can I actually show 2 questions to my customer-- One on the product quality and other on customer service quality, both having a 1-5 rating as it shows?

Out of the box, you can't. But there are Apps in the atlassian marketplace which you can use to achieve this.
Is there a way that the survey can appear once the user resolves the ticket?

A workaround for this would be to use an external link like google forms to collect feedback from the customer.
With google forms pre-filled links and Request resolved customer notification in JSM you should be able to tie the survey to a Jira ticket.
