Good Morning,

Is there a way to add more than one "Summary" field when managing the request form? It seems I can only select it once. In the picture I have inserted, you'll see there is only one listed as "Field 1". I have already gone into project settings and request types but I am only able to add it once.

If you are looking for adding Custom Fields, here is the flow:

Project settings → Fields → Click on "Edit Fields" on the top right of "Actions" button → Then we see Custom fields on the left navigation → Now we see "Create Custom Field"

Project settings → Issue Type → Click on the any of the Issue Type → On the top right you see [Workflow / Fields ] → Fields is where you make the changes of adding an additional field or moving things up and down.

btw (By the way): I am using cloud version of JIRA.

@Eric.Ha : Please take a look at my other comment regarding the Custom Fields. Another constraint that JIRA has is with regards to number of Summary fields. So may be creating a custom field would be an alternative for you.
