How to add additional fields to service requests

How do I add additional fields for Service Requests?  Where is "Due Date"?

You have to first add the "Due date" field to the create screen for that project.   Until that happens, you won't see that field as an option to be added to a specific request type.  There are a couple of different ways to do this, but I think the best way to achieve this is

  1. In Jira, navigate to Cog Icon -> Issues -> Field Configurations.  
  2. Then select "Configure" on the field configuration that is being used by your service desk project.
  3. Find the field called "Due Date" here, then select the link "Screens" for that field.
  4. Make sure to add this field to the create screen of all the request types you want to see this on, and then click update at the bottom
  5. From there you can then return to request types, and edit the fields as in your screenshot.  You should then be able to add that field to the request type.
