Q1: I can't locate the "Issues Raised From Comments" section. Where is this?. I have fisheye/crucible 3.8.0. Was it introduced in a newer release?
Q2: I've noticed that if a JIRA ticket can't be created "inline" (because, for example, the issue has mandatory fields) crucible doesn't make any reference to it, nor JIRA creates the link back to crucible. Is this expected?

In here shows the new JIRA issue that I created from the comment. The comment that originated the issue then also shows the issue key and status under it.
