embed a jira form from the portal to confluence

Is there any way to embed live data from a Jira portal "form" request 

example 

  1. submit a meeting request form with action items in the Jira cloud portal
  2. ticket created in Jira with form notes embedded  
  3. multiple requests for action for a monthly meeting 
  4. compiled requests embedded with form notes summary for each ticket 
  5. automation to the confluence page for meeting agenda composed of all the requests and the following fields 

request #, summary, description, reporter, and date fields 

we need this pretty urgently if anyone can help or point to add on

It sounds like Elements Publish could meet your needs. The app allows you to configure the information you want from a Jira issue to be inserted onto a Confluence page, and you turn your Jira issue into a Confluence page. The Jira data can be synchronized, so changes in the issue are automatically updated on the Confluence page, meeting your live data need. (Btw, I'm with the company behind the app).

Could you expand on your example about the multiple requests and adding to the Confluence page all the requests? I'm not sure I understand that part of your example.
