Table of Contents Macro - how to fully control include exclude features and links

The table of contents macro lists most of the headings I want, but it only made links to several of them. How do I make sure all bullet points have links?

Also, I am confused by the include/exclude features. The Sun's Regex doc didn't help.

As to the lack of hyperlinks, what version are you running?

As to Include/Exclude Headings, if you didn't want the "Note" section listed, just put Note in the Exclude Headings field, otherwise I find the Wikipedia RegEx article indispensable.

I am using Confluence 5.7.4.  I have tried using the include exclude fields of the macro editor but it either includes everything or excludes everything regardless of what I put into the field.
