Permission screen user auto-suggest not displaying full list of users

I am adding users to a space. As I type in a name, suggestions appear below allowing me to quickly add users.

The problem is that many users do not appear in the suggest list. If I type in the username and click Add, the user is successfully added, but the suggest box is saying "no matches".

Is this an index that needs to be rebuilt? Which one? Where?

Yes, the reindex is a good call for this case. I highly suggest that you try to do a rebuild from scratch:
