Is it possible to create a request page to input information and send out?

Is it possible to create a request page to input information and send out?

In confluence, we want to have a request form where other users can fill in their requirements and then one click to send out to an email address with all filled information. Is it possible to realize by user macro? How may work load might need for this kind of user macro? Thank you in advance.

@Shi_ WenI don't think this is possible on Confluence Cloud. Confluence Cloud does not have user macros like Confluence server.
