Most of the time, the images do not load in edit and non-edit mode. I tried clearing cache, cookies, different browsers, and incognito, but nothing helped.
In my organization, it seems that only I am having this issue.
The image placeholder shows "Preview unavailable".

Check if the image files are a) actually files, b) actually attached to the page (you can verify that on the Attachments page for the specific page.
If it's a link to an image, the image should display providing the permissions (both within Confluence and in the external sources) are set up properly. For example, I copied content, with images, from a Zendesk site. Images displayed correctly... until we put that Zendesk site behind login...
That is unfortunate. Does this happen to all pages with all images? Have you tried to change the format of the image. In my case, sometime it happen when I simply copy paste the image from the internet.
I usually resolve the issue by downloading the image, making sure it is png/jpeg and it fixed the issue. Although the behaviour is different for me as the image could be seen, but only for me and other user accessing the page will see the error message you mentioned.
