How to generate report to identify when the document was moved from one state to another

Hi,

I need to generate a report based on which I can get details of movement of document from one state to another i.e. person initiated the document and submitted for approval of another on X date, second person approved on Y date and sent to third person who approved on Z date.

Hope this clarifies the requirement.
