How to Create a Form in Confluence?

I would like to create a form similar to what can be done in Sharepoint. How is this possible in Confluence?

Same as Jason, depends on what you want to have happen after the form is submitted. Another option is Run Plugin for Confluence. It is often used to provide parameters to the SQL macro, script macro, reporting macros, or similar.

Depending on what you are trying to do, a simple template might work for you. Or you might check out the Scaffolding plugin. But if you need a real "submit form" type of functionality, I believe you can embed a Wufoo form.
