I would like to add a link to the top menu bar of our Confluence server instance, like you can see in the next image (this is no real link, but a manipulated screenshot):
What is the best way to get this done?

You'll need an app that can insert a "web fragment" (unless you want to amend the code of Confluence itself).
For a link like that, you'll probably need to write your own app to do it, unless you are happy to install one of the few apps that allows customisable web-fragments to be added (Scriptrunner is the only one I know of, but there may be others)
