How can I add a checklist with labels coming from a confluence database?

Need to create a form with a checklist items of which are listed in a field of a confluence database. Has anyone tried to invent this wheel?

You may need to utilize third-party apps like Elements Connect, Table Filter and Charts for Confluence, or ScriptRunner, set up a Confluence database with the checklist items you want to use, ensuring you define your data clearly using appropriate field types.
